No two sides of the face are perfectly identical, and small differences between the lips are extremely common. However, when one side of the upper or lower lip appears noticeably higher, thinner, fuller or differently shaped, some people may consider lip asymmetry correction.
The right correction depends on what is creating the imbalance. Lip asymmetry may be present naturally, develop with ageing, follow an injury or surgery, or become noticeable after a previous cosmetic treatment.
This distinction matters because adding filler is not automatically the solution. Some patients may benefit from carefully planned volume correction, while others may need a different approach.
The objective should be to create better proportion and harmony, not perfectly identical lips.
Start by Looking at the Lips in Two Situations
Lip asymmetry should ideally be assessed both at rest and during facial movement.
Why?
Because the lips are dynamic structures.
At rest
One side may have less volume, a different border or a slightly different height.
While smiling or speaking
The difference may become more noticeable because muscle movement influences the position of the lips.
An asymmetry caused primarily by volume difference may therefore require a different approach from one caused mainly by movement.
Why Can Lips Look Uneven?
Several factors can contribute.
Some people naturally have asymmetric lips from early adulthood. In others, ageing may gradually change volume and tissue support differently across the two sides.
Other possible contributors include:
- previous injury,
- scarring,
- previous surgery,
- dental or jaw relationships,
- facial nerve or muscle differences,
- and previous filler placement.
Identifying the likely reason is more important than immediately deciding how much filler to inject.
The Three-Part Lip Assessment
A useful way to understand uneven lips is to separate the concern into three components:
Shape
Does one side have a different outline or border?
Volume
Is one portion visibly thinner or fuller?
Movement
Does the difference mainly appear when smiling or speaking?
Sometimes more than one component is involved.
This assessment helps determine whether adding volume is appropriate.
When Can Lip Filler Help?
Dermal filler may be considered when asymmetry is related partly to differences in lip volume or contour.
Rather than adding the same amount to both sides, correction may require different amounts in different areas.
This is an important principle.
If one side is already fuller, adding equal filler to both sides may preserve or even exaggerate the original imbalance.

Does Lip Asymmetry Always Need Filler?
No.
If the difference is caused mainly by muscle movement, scarring, structural anatomy or another underlying issue, simply adding volume may not correct the problem.
In some cases, additional filler could make the lips appear heavier without addressing the true source of asymmetry.
Treatment should therefore follow assessment rather than the other way around.
What If Previous Filler Made the Lips Uneven?
Previous filler can occasionally contribute to asymmetry because of differences in placement, volume, tissue response or product distribution.
Before additional filler is considered, the clinician should evaluate:
Where is the existing product? → Which area is actually uneven? → Is more volume needed? → Would adding more make the lips too full?
Automatically placing more filler into an already treated lip is not always appropriate.
Can Perfect Lip Symmetry Be Achieved?
Perfect symmetry is not a realistic treatment promise.
Natural faces contain subtle asymmetry. The upper and lower lips also have different shapes, proportions and movement patterns.
The aim is usually to make a noticeable imbalance less visually distracting while maintaining natural facial expression.
Why Does Facial Balance Matter?
Lips should not be evaluated as an isolated feature.
Their appearance is influenced by the:
- nose,
- chin,
- teeth,
- jaw,
- smile,
- and overall facial proportions.
Making the lips dramatically fuller simply to compensate for asymmetry may create a new imbalance.
Conservative correction can therefore be more appropriate than pursuing maximum volume.
What Happens Immediately After Lip Filler?
Temporary swelling can make the lips appear uneven during early recovery.
Mild tenderness or bruising may also occur.
This means the appearance immediately after treatment is not necessarily the settled result.
Patients should follow their treating clinician’s aftercare instructions and allow appropriate time for early swelling to improve before judging symmetry.
When Should Uneven Lips Be Medically Evaluated?
Long-standing mild asymmetry is different from a sudden change.
New or sudden facial or lip asymmetry, especially when associated with weakness, numbness, speech difficulty or other neurological symptoms, requires prompt medical assessment rather than cosmetic treatment.
Cosmetic correction should only be considered after an appropriate cause has been established.
What Are the Risks of Lip Filler?
Potential complications can include:
- swelling,
- bruising,
- tenderness,
- asymmetry,
- lumps or irregularities,
- infection,
- undesirable volume,
- and vascular complications.
Some filler complications can be medically significant and require prompt treatment.
This is one reason injectable procedures should be performed by appropriately trained medical professionals.
A Better Goal Than “Perfect Lips”
A useful treatment plan asks:
What is creating the asymmetry?
↓
Is it present at rest, during movement or both?
↓
Would changing volume actually address it?
↓
How much correction maintains natural proportions?
This approach is more useful than simply attempting to make both sides mathematically identical.
Correction Should Preserve Expression
Lip asymmetry correction should focus on balance rather than volume alone.
Some patients may benefit from subtle contour or volume correction, while others require evaluation of movement, scarring, previous treatment or underlying facial structure.

FAQs
Many forms of lip asymmetry can potentially be improved, but the treatment depends on the underlying cause.
Filler may help when differences in volume or contour contribute to the asymmetry.
Not necessarily. Treatment is based on individual anatomy.
Temporary swelling can initially create asymmetry. Persistent differences should be assessed after appropriate settling.
Perfect facial symmetry cannot be guaranteed.
Yes. Muscle and nerve function can influence how the lips move during smiling and speaking.
